Stanford Chronic Conditions Program*
If you are a senior with a chronic condition who would like to learn to live a
healthier, more comfortable life, this program is for you.
Developed by the Stanford School of Medicine, "Seniors Take Charge: Stanford
Chronic Conditions Self-Management Program" will help you more effectively
manage fatigue, pain, anger, and depression. The six-week program also supports
eating healthier and developing and maintaining a manageable exercise routine. The program meets once a week for six weeks.
